Game Reports

Adult games

In order to file on-line reports for adult games you must obtain an ID number and code from Sherrie Griffiths.  To file a report, you will need to go to the Utah Soccer Association site at  www.utahsoccer.org;  Only games where no player passes have been retained by the officials or had no incidents may be filed on-line.  Game rosters from those games must still be forwarded to the Utah Soccer Association office and must be done on a weekly basis. 

Any game where player passes were retained for any reason, a written game report must be filed and received at the Utah Soccer Association within two (2) days of the game. 

THE OLD LEGAL SIZE GAME REPORT IS NO LONGER VALID.  PLEASE THROW THIS REPORT OR ANY COPIES AWAY IMMEDIATELY.  THEY ARE NO LONGER ACCEPTED.   THE NEW FORM WHICH WAS ISSUED OVER A YEAR AGO, IS THE ONLY FORM TO BE USED!  This form comes in two parts and is letter sized.  The second part is a supplemental game report and is only necessary for additional information. 

All ejections must include a detailed explanation of the infraction.  It is mandatory that game officials keep track of the goal scorers during each game and must be reported on the game report.

Youth Games
